This form is for families who are ready to enrol.

Please use this form if:

• You would like to enrol directly into a Stage 1 Discovery Program (no trial lesson), or

• You have already enquired and/or had a trial lesson and now wish to confirm enrolment, or

• You are a current family pre-enrolling / re-enrolling for the next term or new year so we can hold your place and lesson time.

If you have more than one child, please submit one form per child.
